You can claim from Assupol by either submiting your claim online on the Assupol website , download and complete the claim documents, and submit on Email: claimsregistration@assupol.co.za; or Fax: 087 233 6300/7300; or At your nearest Assupol branch.
Documents Required
To submit a death claim you require the following documents:
- Completed death claim form ( Download);
- Certified copy of the original death certificate;
- Certified copy of the deceased’s ID with deceased or similar notice stamped on it;
- Copy of the three page Department of Home Affairs- BI 1663 (Notification of death/still-birth) form;
- Certified copy of the claimant’s ID (the person submitting the claim); and
- Bank statement showing the account number and account holder details.
For Withdrawal, Maturity, Cashback and any other claims using any other medium
- A completed claim form for Withdrawal / Maturity / Cashback (available on our website);
- If you are not the policyholder then also a certified copy of the claimant’s ID;
- If your premium is paid from your salary (PERSAL) or if you want the money to be paid to another account instead of the one we have on record (the one your monthly debit order goes off from) then you will need a bank letter that confirms the account holder details (this you can get from your internet banking/banking app, which you can email to yourself and then to us, or an ATM that prints statements, or your nearest bank branch).
